Become a Carelon Health Advanced Primary Care patient through a Medicaid plan to enjoy personalized, whole person care. Here are five ways you can experience the Carelon Health difference:
- “We’re listening.” Our doctors and care team take extra time to listen to your concerns and provide a personalized care plan.
- Care from anywhere. Our team will meet you in our Carelon Health Care Centers or virtually, often the same day.
- A head start on better health. During your first Healthy StartTM head-to-toe exam, we get to know you and your whole health. We’ll create a personalized care plan to help you get the care you need to live your healthiest life, including managing any ongoing health conditions.
- We speak your language. Interpreter services — including sign language — are available in our care centers at no cost to you so you can easily talk to our doctors and staff.
- Community and connection. Our Carelon Health Care Centers host social events and educational sessions with doctors and staff. Check with your care center for details.

This head-to-toe checkup will create a strong foundation that will allow us to design care around your needs, concerns, and goals. You’ll receive a personalized care plan that will outline how we can help you become healthier and more resilient every day.
