Carelon Health is working with independent insurance agency eHealth to help you compare your Medicare coverage options. You can search for plans that include Carelon Health by using the online search tool here.

Please have the following information on-hand when contacting eHealth:

  • Names of your doctors (New Patients: You must include a Carelon Health doctor as your PCP, you can find a provider here)
  • Current prescription drug names and dosages
  • Your Medicare ID card
  • Your Medicaid information, if applicable

Check your Medicare eligibility

Most people become eligible for Medicare during the Initial Enrollment Period — three months before they turn 65, the month they turn 65, and three months after they turn 65 (seven months total). 

You may be able to sign up sooner if you have a disability, end-stage renal disease, or amyotrophic lateral sclerosis (ALS, also known as Lou Gehrig’s disease).
